Aims/Description: This module will support the principles and practice of supporting the person with chronic heart failure and their family. Principles of epidemiology, pathophysiology, assessment, pharmacological and non-pharmacological management, social support and palliative care will be included. Service delivery strategies will be discussed. This module will support the CHD National Service Framework goals for heart failure.
